This form is to be used to collect information that will be needed when damages/violations are reported in the Connecticut Call Before You Dig DamageAccess system. This form should NOT be sent to PURA or CBYD. The information collected on this form must be input into the CBYD DamageAccess online reporting system, available at www.cbyd.com.
